APPENDIX 5 NOTIFICATION LETTER NOTIFICATION REQUIRED BY ó

6-305 (a)(2) OF THE RULES June 18, 2012 To Whom It May Concern,

Lawrence Young of Cambridge Ver­mont, has submitted an application to the Vermont Agency of Natural Resources (Agency) for certification of a solid waste management facility for land application of septage. The Agency has determined that the ap­plication is administratively complete and is proceeding with its review process. The project consists of ap­plication on land owned by Robert and Linda Sweet in Fairfax.

The Agency’s review process allows the public and interested parties the opportunity to comment on the appli­cation at two times. The first (this notice), occurs when the Agency has determined that the application is administratively complete. The second opportunity for public com­ment will occur when the Agency has completed its technical review of the application, determined that it demonstrates compliance with the Solid Waste Management Rules (e­ffective March 15, 2012), and has prepared a fact sheet and draft certi­fication for the project. Having now determined that the ap­plication is administratively com­plete, the Agency hereby solicits via this notice, comments from the pub­lic on the materials submitted in support of the application. The De­partment will accept comment on the application materials at the ad­dress provided below until 15 days after this notice is printed in the newspaper.

The applicant, the Northwest VT So­lid Waste Management District, Northwest Regional Planning Com­mission, District Environmental Commission, and the Town of Fair­fax will automatically be provided with the application materials, the fact sheet and draft certification, and notice of both comment periods as required by the Rules. Other inter­ested parties who wish to automat­ically receive notification and copies of the fact sheet and draft certifica­tion must request such notification in writing to the Agency at the address provided below. Unless this request is made in writing, notification of the availability of these documents and the associated public comment peri­od will be made only through legal notices in the Burlington Free Press and the St. Albans Messenger.

A public informational meeting can be called for the second public com­ment period by petition of twenty-five (25) or more residents of the af­fected municipality(s), by request of the governing body of the affected municipality(s), or at the discretion of the Agency. Petitions or requests for a public informational meeting must be submitted prior to the end of the second public comment peri­od to the address provided below.

It is located on Bob & Linda Sweet's land at the South side of the cornfield - You can't see it from the Fletcher Road or my house.
