Rescue Personnel Debbie Spear, Peggy Matthews, Sharon Isvak and James Kurfix were availailable to help seniors with questions and to fill them out

And this is a decal to put on your front door so Rescue will be aware you have a file of life

The above form filled out preferably in pencil so that you can erase and change if necessary is folded and put in the magnetic packet on your refrigerator


Back side shown above should also be filled out.

Two copies are in the packet so one can be kept in your purse or wallet in case something happens when you are away from home
